If you plan to travel, please download and complete our pre travel assessment form at least 6 weeks before your travel date. Once complete, please hand in to the Practice. A Nurse will then contact you to arrange an appointment.

Most travel vaccines can be administered free of charge through the NHS. However, a few can only be obtained on a private prescription, the cost of which is paid to the Pharmacy by the patient. These include:
- Anti Malarial
- Hepatitis B
- Japanese Encephalitis
- Meningococcal
- Rabies
- Tick-Borne Encephalitis.
Administration of these vaccines also incurs a charge of £40.00 to the Practice, as well as the private prescription charge.
Unfortunately we are not authorised to administer the Yellow Fever vaccine. However, if this is indicated the Nurse will be able to advise you how to obtain it
